По данным МСКТ и УЗ измерениям скорости построены пять персонифицированных моделей области проксимального анастомоза бедренной артерии после операции бедренно-подколенного шунтирования. Проведены расчеты пульсирующего ламинарного течения несжимаемой ньютоновской жидкости. Сопоставление расчетного векторного поля скорости для одной из моделей с данными клинических измерений методом ультразвуковой высокочастотной векторной визуализации показало удовлетворительное согласование результатов. Проанализированы послеоперационные изменения гемодинамики: - Существенное сужение проходных сечения общей бедренной артерии (ОБА) перед анастомозом и шунта за ним из-за нарастания неоинтимы привело к более возмущенному и неоднородному потоку; значения осредненных по циклу сдвиговых напряжений на стенке (TAWSS) вблизи анастомоза увеличились, а величина индекса их колебаний (OSI) по всей модели уменьшилась. - В результате расширения проходного сечения анастомоза после реоперации пластикой с наложением заплаты вблизи анастомоза значительно уменьшились значения TAWSS; при этом образовалось выраженное вихревое течение и увеличились значения OSI. - Уменьшение доли расхода через шунт из-за увеличения периферического сопротивления ветви шунта при слабом изменении геометрии не привело к значительным изменениям гемодинамики. Во всех моделях выявлена область с высокими значениями TAWSS и низкими OSI в нижней части ответвления шунта от ОБА. С использованием параметра спиральности LNH обнаружено формирование одновихревого течения в шунте, направление которого определяется индивидуальной геометрией области анастомоза.
According to MSCT and ultrasound velocity measurements, five personalized models of the proximal anastomosis area of the femoral artery after femoral-popliteal bypass surgery were built. Calculations of the pulsating laminar flow of an incompressible Newtonian fluid were carried out. Comparison of the calculated velocity vector field for one of the models with the data of clinical measurements by ultrasonic high-frequency vector imaging showed satisfactory agreement of the results. Postoperative hemodynamic changes were analyzed: - A significant narrowing of the femoral artery (FA) before the anastomosis and the shunt behind it due to the neointima overgrowth led to a more disturbed and heterogeneous flow; the values of the time-average wall shear stresses (TAWSS) near the anastomosis increased, and the value of oscillatory shear index (OSI) decreased throughout the model. - As a result of the expansion of the flow section of the anastomosis after plastic re-surgery with the application of a patch the TAWSS values significantly decreased near the anastomosis; at the same time, an intensive vortex flow was formed and the OSI values increased. - A decrease of the ratio of the shunt flow rate due to an increase in the peripheral resistance of the shunt branch with a slight change in geometry did not lead to significant changes in hemodynamics. In all models, an area with high TAWSS and low OSI values was identified in the lower part of the shunt branch from FA. Using the LNH helicity parameter, the formation of a single-vortex flow in the shunt was revealed, which direction is determined by the individual geometry of the anastomosis region.
